The Web Platform allows you to create fillable Webforms on your site. Please note that, if you are creating Webforms, your forms must meet UCLA Health's privacy guidelines.
 

  1. Log in to your site.
  2. Hover your mouse over Structure
  3. Hover your mouse over Webforms.
  4. Click Overview.

     
  5. Click + Add webform.

     
  6. Add the Title of your Webform.
  7. Click Save.

     
  8. Click + Add element.
  9. Select the desired element(s) for your form and complete any necessary fields depending on your selection.  
  10. If you would like your form to have multiple pages, click the + Add page button and add any needed elements.
  11. Scroll to the bottom of the page and click Save elements.
  12. Click Webforms to return to the page with all available Webforms.


     
  1. Log in to your site.
  2. Hover your mouse over Structure
  3. Hover your mouse over Webforms.
  4. Click Overview.

     
  5. Find the Webform you would like to update and click Build.


     
  6. Make desired changes to your form.
  7. Scroll to the bottom of the page and click Save elements.
  8. Click Webforms to return to the page with all available Webforms.

 

  1. Log in to your site.
  2. Click Structure
  3. Click Block layout.


     
  4. Scroll to Footer Bottom.
  5. Select Place block.


     
  6. Locate the Webform block and select Place block.


     
  7. If you would like the title of your Webform to be displayed, check the Display title.
  8. Type the name or title of the desired Webform.
  9. Click Save block.


     
  10. Scroll to the bottom of the page and click Save block.

     
  11. Your Webform will now appear in the footer at the bottom of every page.
